Course: MD ID 7110 - Layers of Medicine: Navigating the Hierarchy in Medicine

Description: The Layers of Medicine series of courses is an ongoing dialogue about the interdisciplinary complexities and challenges of health care. This dialogue is rooted in both the very large and the very small. We will attend to the 'big picture' conceptualization of what medical care means and the broader socio-cultural factors that shape its delivery. At the same time, we will attend closely to the unique particularities of the patient-doctor relationship: analyzing ethically and practically challenging scenarios and examining issues of professionalism in medicine.

The material covered in this elective course follows topics and approaches covered in the required LOM courses, which can be thought of as foundational "layers" of knowledge necessary, alongside scientific and clinical courses, for delivering best and most appropriate care. This elective provides a more intensive exploration, thus extending our geological metaphor to "digging deeper" into what those foundations hold.

Section 1: Navigating the Hierarchy in Medicine
January 29 -- ????, Thursdays, 4-6PM
Section Description: This is a Layers of Medicine elective designed for students who have started their journey from the classroom to the clinic, though it is open to any interested student. Students will be provided practical guidance on navigating the hidden and overt medical hierarchies with the student/resident at the center of a complicated web of senior residents, attendings, hospital administration, the medical profession and medical specialty organizations, insurance companies, as well as society at- large and government. Students will also be provided with guidance on how to navigate person-level hierarchies (e.g., how to build a strong recommendation with residents and attendings), while also learning about how to spur change within the new systems in which students will find themselves (e.g., hospital systems, insurance companies).

Section 2: Radical Listening at the End of Life
March 22 -- April 23, synchronous meetings, Tu and Th, 6-8PM
Section Description: This is a Layers of Medicine elective designed for students who have started their journey from the classroom to the clinic, though it is open to any interested student. Medical training focuses on the preservation of health, which may seem to imply that care of patients deemed medically futile and/or dying is beyond the purview of physicians. In this course, students will use narrative to re-examine end-of-life care and ethics. Reading and discussing narratives by and about diverse patients and professionals, students will learn how to "think with" stories. Through writing about their own experiences, students may better understand the social isolation and fear of dying patients, and through this "radical listening," redefine their role in helping patients live meaningfully to the end.

Section 3: Leaning into Growth
April 5-9, synchronous meetings, M-F, 1-5PM
Section Description: This is a Layers of Medicine elective designed for students who have started their journey from the classroom to the clinic, though it is open to any interested student. Medicine is a demanding and ever-changing field that requires skill at navigating uncertain, and sometimes upsetting, situations on a regular basis. This section will focus on not only accepting discomfort, but also transforming it, by reflecting on beliefs and attitudes, reframing goals, and practicing effective communication. Learning these skills in training will help students succeed in their future, not only in their practice, but also in personal fulfillment in their careers. This section will address topics like patient-provider conflict, medical errors, racism in medicine, and other situations that require self-observation, reflection, and individual and systemic approaches to a growth mindset.
